Heard’s restraining order filing describes two recent incidents where she claims Depp was drunk, high, and abusive.

Actress Amber Heard filed for divorce from Johnny Depp last week, as well as a restraining order against him,claiming that the actor’s substance abusecontributed to him physically and verbally abusing her throughout the course of their relationship. “During the entirety of our relationship,” the actress declared in court papers, “Johnny has been verbally and physically abusive to me.”

“He has a short fuse,” she added in the statement. “He is often paranoid and his temper is exceptionally scary for me as it has proven many times to be physically dangerous and/or life-threatening to me. Johnny relationship [sic] with reality oscillates, depending on his interaction with alcohol and drugs. As Johnny’s paranoia, delusions and aggression increased throughout our relationship so has my awareness of his continued substance abuse. Because of this, I am extremely afraid of Johnny and for my safety.”

A judge temporarily granted Heard’s request for a restraining order against the 52-year-old actor, who she was married to for 15 months, after she showed up to court with a black eye she says Depp caused by throwing an iPhone at her face. A photo of her bruised face has circulated the Internet. Heard also said in her statement that there was a “severe incident” last December when she “truly feared that my life was in danger.”

Heard’s restraining order filing describes two recent incidents in alarming detail, starting with the evening of April 21, 2016—when Heard was celebrating her 30th birthday at the couple’s apartment. She claims Depp showed up drunk and high as people were leaving the party. After an argument, according to Heard, Depp threw a champagne bottle at the wall and a wine glass on the floor, then “grabbed me by the shoulders and pushed me onto the bed, blocking the bedroom door. He then grabbed me by the hair and violently shoved me to the floor. Johnny was also screaming and threatening me, taunting me to stand up.”

In the second incident, Heard said Depp returned home drunk and high on Saturday, May 21, the day after his mother had passed away. She claims he became angry and jealous about her friendship with photographer and LGBT activist iO Tillett Wright. “As Johnny continued to rant in an aggressive and incoherent manner, he then demanded we call our friend iO Tillet Wright to prove his paranoid and irrational accusations about some delusional idea he was having,” Heard claims.

After calling Wright, Depp allegedly threw the cellphone at her face then “forcibly pulled back my hair as I attempted to stand up from the sofa. I then yelled out ‘Call 911’—hoping it would be heard by iO who was still on the phone. Johnny continued screaming at me, puling [sic] my hair, striking me and violently grabbing my face.”

Heard and Depp’s neighbor, Raquel Rose Pennington, allegedly entered the apartment and attempted to intervene. Pennington claims that Depp was being extremely abusive and violent. “Many times over the past few years,” Pennington added in her court statement, “Amber has confided and complained to me about Johnny’s abusing her, both physically and verbally.”

The judge granted Heard’s request for a restraining order—effective until June 17—but denied her request for $50,000 per month of spousal support as well as her request to send Depp to 52 weeks of court-ordered anger management classes. The next court hearing is scheduled for June 17.

Depp’s history of substance abuse has been widely reported, and he has spoken out in the past about his tendency to use alcohol as a coping mechanism. In 2013, the actor said he was staying sober during his divorce from singer Vanessa Paradis, because drinking over their break-up “could have been fatal.” Back in 2012, there were reports that the actor was drinking heavily as the couple’s marriage was falling apart.

In 2014, the actor made headlines when he appeared intoxicated during a speech at the Hollywood Film Awards. And an “insider” told a tabloid that Depp had realized “that he clearly had an issue” with alcohol, and had been traveling with a sober companion to help him stay off booze.
